Hair in nature
exists in natural groupings. These groupings,
known as follicular units, may contain
1 to 4 hair. To mimic nature and to achieve
the most natural and undetectable results
in hair restoration, we utilize solely
follicular unit graft(FUG)transplantation.
The modern hair restoration technique
has shifted towards using all FUGs instead
of the old minigrafts(containing 3-6 hair)which
are intrinsically pluggy looking in nature.
Hair loss in androgenetic
alopecia is an inherited trait. It is
a life long process and progressive in
nature. Every single hair in the scalp
is predetermined with a program to keep
growing or to fall ultimately. The factors
involved are (1) genetic (2) your natural
hormone DHT (3) timing.
The hair in the front and top of the scalp
have been genetically programmed to fall
out under the influence of DHT at a specific
time in a person's life.
The hair at the sides and back of the
scalp are less influenced by this hormone
and will continue to grow throughout the
person's life . The transplantation of
this hair (back and sides) to a bald area
does not change its ability to grow. This
forms the scientific basis for the success
of hair transplantation in which hair
is taken from the back and moved to the
area of hair loss. Candidates for a hair
transplant are those individuals with
hair loss who have sufficient donor hair
from the fringe areas of the scalp to
transplant to the balding areas.
The side effects
of hair transplantation are minor consisting
of mild pain and discomfort after the
operation, swelling, and scab formation.
All these are transient and subside within
a couple of days.Serious problems of bleedings,
scarring and infection are rare. Modern
hair transplantation is comfortable, results
are predictable , and the outcome is satisfactory
to most people.
Our Technique
Hair transplantation
is a process in which the existing hair
follicles are redistributed to the balding
areas. Hair transplantation will not increase
the number of hair follicles. Substantial
transaction (waste) of follicles may occur
during donor harvesting, donor strip slivering,
grafts cutting for production, and implantation
(insertion). Blind cutting of donor strips
during harvesting and cutting the strips
for graft productions cause most of the
wastage. This waste rate varies from 5%-37%
reported in the medical literature. Preserving
the precious donor follicles without waste
poses a great challenge to transplant
surgeons.
Our clinic makes use of the most advanced
and sophisticated technology to minimize
the waste .
We make use of the open
technique, reported by Dr. Pathomvanich,
for donor harvesting. It is a technique
in which hair follicles are clearly visualized
with the help of loupes magnification
during donor harvesting. The waste rate
with the method is lower than 2%.
The second step involves
slicing the donor strips into tiny slivers
under 10X magnification with the use of
the CCCD video camera magnification system
reported by DR. Melvin Mayer and DR. Sharon
Keene during the ISHRS hair meeting in
Vancouver .The final grafts cutting and
production are carried out with the help
of stereoscopic microscopes.
It is time consuming
to carry out the whole preparation. However,
it is worthy of doing if we can keep our
transaction (wastage) rate below 5% .

Q:Is hair transplantation safe
?
A: It is a safe procedure
since it involves only the superficial
layer of the skin under local anesthesia.
General anesthesia with its inherent risks
is not used.
Q:Is hair transplantation painful
?
A:Most patients feel
the sensation of an “ant bite” during
the initial injections of local anesthesia.
After that, it is virtually painless.

Q:Is the new
hair permanent ?
A:The donor hair at the
back are not affected by the hormone DHT
.Once transplanted to the balding areas
, it retains the same properties and will
start growing permanently .
Q:How long does
it take for the new hair to start growing
?
A:Hair grows at the rate
of about 1 cm per month . For the first
3 months after the procedure , you do
not see any hair .Three or four months
later new hair will start growing and
becomes noticeable.
Q:Will the procedure
damage my existing hair ?
A:After the procedure,
your implanted hair will initialy fall
out. Don't panic, the root remains healthy.Your
existing hair might also fall out as a
result of temporary vascular impedance
during the procedure . Your hair might
look even thinner than before the procedure.
All this hair will come back several months
later .
Q:How may transplant
sessions do I need ?
A:In general 1-2 sessions
will be enough . The more extensive the
areas of baldness and the more density
you want , the more sessions will be required.
You should wait for at least 6-12 months
between sessions.
Q: Will the
scar at the back of my head look “ugly”?
A: It is just a small
fine linear scar that is generally well
hidden by your surrounding hair .
Q:Will I need
to take time off my work ?
A: Most patients resume
daily activities the next day after surgery
. Mild exercise can be resumed after 7
days and normal exercise after 10 days
.
Q:What are the
complications ?
A: The side effects are
minor, consisting of mild pain and discomfort
, swelling ,and the formation of scabs
after the procedure . All these are transient
and will last only a few days . Numbness
of the scalp will occur, but will disappear
after several months.

Medical
Treament
Finasteride or Propecia blocks
the conversion of testosterone to DHT.
It does this by inhibiting the action
of type II 5-alpha reductase. Finasteride
is recommended only for men and has been
shown to produce a decrease in the rate
of hair loss in 88% of patients and some
hair growth in 66% if used for 6 months
or longer.
It is not
recommended in women, as there is a possibility
that teratogenecity of the male fetus
during pregnancy might occur. This drug
works well in the vertex but not in the
frontal areas. Side effects include sexual
dysfunction in less than 2% of patients
which is reversible when the drug is stopped.
Propecia is metabolized in liver; patients
with liver function impairments are advised
against taking the drug. Long term side
effects are not known at this time. It
seems reasonable for patients who are
not ready for surgical intervention to
use Finasteride as a temporary “stopgap”
measure for 1 or several years, or until
such time that circumstances are suitable
for surgical treatment, especially if
the treated area is the vertex .
Rogaine
is the brand name for the topically applied
Minoxidil solution; 2% solution for women
and 5% for men. The effect of this medication
is to increase the quality of hair by
increasing the diameter and length of
fine miniaturized hair.
It does
not grow hair in areas that are bald.
Studies show that hair grows in the vertex.
It does not create hair growth in the
frontal area. If Rogaine is going to work
for you, it may take 6-12 months before
you see any results, and it will have
to be taken indefinitely. The medication
has to be used twice a day for maximum
benefit.
Once the
drug is stopped, the previous pattern
of hair loss resumes. There are essentially
no systemic side effects, although some
patients may complain of scalp irritations
and heart palpitations or light-headedness.
